How can I find out what or where to obtain a proof of service form?

I applied for my CPL and was denied because of a seat belt ticket back in 2002. the ticket was put on my bankruptcy case in 2010. i thought it was taken care of until i received the letter of denial. It stated i was a fugitive of justice. So i saw the magistrate and took care of it. I then e filed my appeal, but I received another letter stating I need to fill out a proof of service form and file it with my city clerks office. The problem is they have no clue as to what I’m talking about, seeing that the laws have changed and they are just learning their selves.

This is your CPL and way of making a living, hire a good attorney in Detroit to figure this out. Your seat belt case has nothing to do with discharge in bankruptcy. Have your attorney call and ask you what form they want to get you on your way back to making a living in your chosen profession.
