License suspended in NJ due to multiple (2) diff license due to mistake in form for eye colour…..in 1-black and in 2nd brown..

Hi,
I was having license in 2007-2008 and 2010-2011 in new jersey….but by mistake eye colour I mentioned is black and brown in 2 diff application …so they have issued 2 diff license number……so in 2014 octomber they sent notice regarding multiple license number and eye colour difference. And in November they suspended license.but it was upto 2010 to 2011 only. I haven’t got the notice of 2014 as I was moved to AZ

But after 2011 I was moved to Arizona and had AZ license and then in 2012 had MA license for 5 year.
Now again I moved to AZ and when I went for license transfer I came to know all this due to I am not eligible in NJ and MA so AZ also denied and gave me letter to contact Nj and MA dmv.
So now what to do remove suspension in NJ and how to get original notice they sent in 2014 so that can come to know written reason and which other agency I need to report about reinstantment.
